What You Can Do in the Assessments Section of the Diagnosis Tab

The Assessments section gives clinicians a dedicated workspace to create, review, and manage all assessment‑related documentation for a client. From this tab, you can:

  • Create a New Assessment Start a fresh assessment using any template built under Account Settings → Custom Notes → Assessments. You’ll select the Assessment Type and enter the Assessment Date before completing the form.

  • Save Assessments as Draft or Completed Choose Save as Draft if you’re still working or Save & Mark as Completed when the assessment is finalized. All assessments—drafted or completed—automatically appear in the list view.

  • View All Completed and Drafted Assessments The default page shows a list of all assessments for the client, including Assessment Date, Professional, Assessment Type, Status, and available Actions.

  • Open, Edit, or Review Past Assessments Click any assessment in the list to reopen it, make updates (if not completed), or review previously completed documentation.


Navigate to the Diagnosis tab in the client chart or from their session note and select the Assessment section. Then, select New Assessment to start a draft. 



When you begin a new assessment, the selected Assessment Type template will load automatically. You’ll be able to choose from a dropdown list of any custom assessment templates created within account settings - custom notes.

Once the assessment is open, you can complete the fields as needed and choose how to save your work:

  • Draft status– keeps the assessment editable and stored in the Draft status.

  • Completed status – finalizes the assessment and marks it as Completed (this is still editable)

Once the status has been chosen, you can select Create Assessment.



When opening a draft assessment, you can make updates and/or change the status to Completed and select Save Changes.



Assessments can also be shared for supervisor review by checking 'Ready for Supervision'
