Please note: this template was developed based on our knowledge of how neuropsych evaluations are billed to most insurance companies (either for in-network claims or for out-of-network superbills). However, this is only to be used as a guide. We cannot guarantee that the structure of these codes will work for all insurance companies. 
We've created the following document to help practices develop a process for billing neuropsych evaluations within MBP: MBP - Neuropsych Billing & Scheduling Template

To create your own copy, click on "File" in the menu bar and then select "Make a copy."

At the bottom of the document, there is another tab, which depicts an example of how the template will operate once you've calculated and entered the HR (hourly rate) and HHR (half hourly rate) into the proper cells. You can view a screenshot of that here:
