Create and Manage Client Inquiry Forms

Client Inquiry Forms allow practices to collect prospective‑client information directly through My Best Practice. These forms help streamline intake, reduce manual data entry, and ensure all inquiries are captured in one centralized location.

What Are Client Inquiry Forms?

Client Inquiry Forms are customizable forms that prospective clients can complete when reaching out for services. Once submitted, the information flows directly into MBP, allowing your team to review inquiries, follow up, and convert prospects into active clients.

These forms are ideal for:

  • Website contact pages

  • Social media links

  • Email outreach

  • Community referral sources

How to Create a Client Inquiry Form

  1. Navigate to Settings Go to Settings → Account Settings → Online Booking.

  2. Select “Edit” under Inquiry Form This opens a blank template where you can build your custom inquiry form.


  1. Add Your Fields You can include fields such as:

    • Name

    • Contact information

    • Reason for seeking services

    • Preferred clinician

    • Insurance details

    • Availability

    • Any additional screening questions your practice requires

  2. Customize Field Types Choose from text fields, dropdowns, checkboxes, and required/optional settings to match your workflow.

  3. Save Your Form Once saved, your form becomes available for sharing and embedding.



Sharing Your Inquiry Form

After creating your form, you’ll see a shareable link that can be:

  • Added to your website

  • Included in emails

  • Shared on social media

  • Provided to referral partners

Anyone with the link can complete the form, and submissions will automatically generate an email notification and appear in your MBP account.

All completed inquiries can be found under:

To Do → Client Inquiries

From here, you can review submission information


Editing an Existing Inquiry Form

Inquiry forms are live, meaning any changes you make will immediately affect the version currently in use.

If your form has already been shared or used, we recommend:

  • Creating a copy of the existing form

  • Making your updates in the new version

  • Replacing the old link wherever it was shared

This prevents disruptions for prospective clients who may already be accessing the original form.

Best Practices

  • Keep your form short to reduce drop‑off rates

  • Use required fields only when necessary

  • Review submissions regularly to avoid missed inquiries

  • Create multiple versions if different clinicians or services require unique screening questions
