Creating Custom Note Template

Step 1: Navigate to Account Settings within the Setting dropdown. 


Step 2: Go to Custom Notes tab then click + Add New Custom Note.



Step 3: In the   Properties   tab, enter the name for the new note template and choose whether you want it to include the following fields: 

I. Homework & Agenda

  1. The Homework field allows you to enter homework assignments that can be viewed and checked off when completed within the client portal. Here, you can also see data on the client's homework compliance rate over time to see how often they're completing their therapy assignments.
  2. The Agenda field allows you to set an agenda for the current session or add agenda items for the next session. If you don't complete an agenda item that you've added for the current session, you can easily press a button to have it auto-populate into the next session's agenda.

II. Symptoms and Interventions

  1. These fields can be customized within your account settings. Learn more about this feature here:

III. Psychotherapy Note

  1. This field is for notes that should be kept separate from the client's medical record.

IV. Diagram

  1. Here, you can draw diagrams directly within the session note.

Step 4: Click on the Design tab to start designing your custom note. Select the elements you want to include in your note from the options provided in the Add Elements box. 

Elements: 
  1. Title - This will create a field where you will input your chosen title.
  2. Text Box - This will create a field for the user to enter a limited amount of text.
  3. Number - This will create a field in which the client can select a number by clicking on up/down arrows.
  4. Date - This will create a field for selecting the date from the calendar.
  5. Text Area - This will create a field input a plain text for the notes.
  6. Rich Text Editor - This will create a field to create custom text and/or provide the user with other practice information.
  7. Checkbox Group - This will create a field in which the user can select MULTIPLE answers from a checklist of options.
  8. Radio Group - This will create a field which the user can select ONE answer from the list of options.
  9. Dropdown - Just like the Radio Group, in this field the user can only select ONE answer from the list of options but in a Dropdown format.
Step 5Once done, click Save Changes
You can now use your custom note template when writing your session notes! In order to do so, simply select the custom note template from the dropdown within a client's session note.
