Once your Claims Center has been configured and the UB-04 functionality has been enabled, you will see that you're able to select UB-04 as an option at the bottom of the insurance plan entry area. After clicking the UB-04 box, the additional fields will populate for selection. Here, you can set the defaults for that specific client's plan. These defaults will then populate on all UB-04 claims for that client when utilizing that specific plan. 

In order to file a UB-04 claim, you'll navigate to that claim within the Pending Claims tab in the Claims Center or within the individual client's Claim Center tab. Then, you'll click "Actions" and select UB-04. 

On the screen that populates, you'll need to complete the bottom fields. 

Once you have entered the desired information, you can click save changes. 

Prior to filing the claim, you can then view a PDF of the actual UB-04 form by clicking View UB-04. 

If everything looks correct, you can close this window and then click "Actions" --> File Claim.
